Pinecone is strongest at scales past where Postgres vector search starts to strain, with little operational work. We reach for it when that is the property a workload actually depends on, and we say so when it is not.
The case for a managed vector database is operational rather than technical: someone else handles index tuning, replication and scaling. That is worth real money once vector volume is large enough that those become full-time concerns.
Below that point the cost is a second system, a second bill, and losing the ability to join vectors against your relational data in one query. Most projects we see reach for it earlier than the workload justifies.
